- Transfers are permitted only to immediate family members, specifically the cardholder's spouse, child, parent (mother or father), or sibling.
- Valid proof of relationship and identification documents must be presented and verified prior to any transfer.
- Upon transfer, the recipient shall be entitled only to the remaining unused benefits associated with the card.
- Benefits already availed by a previous cardholder, including the One-Time Annual Physical Examination (APE), shall be considered consumed and may not be reinstated, repeated, or converted to another benefit.
- The card may not be transferred to individuals outside the immediate family.

4. The cardholder is entitled to (1) Laboratory Package which includes (1) Complete Blood Count (CBC), (1) Urinalysis, (1) Fecalysis (optional based on school requirements). The Laboratory package is non-transferable, non-convertible to cash, and may not be substituted for other laboratory tests, procedures, or medical services.
5. The cardholder is entitled to a fifteen percent (15%) to twenty percent (20%) discount on selected laboratory and diagnostic services offered by the clinic.

5. The Cardholder is entitled to (1) physical therapy or mobility assessment only.
6. The cardholder is entitled to a twenty five percent (25%) discount on selected laboratory and diagnostic services offered by the clinic.
