Terms & Conditions — CarePlus Flex
1. The cardholder is entitled to unlimited Primary Care clinic consultations and teleconsultations during the card's validity period. Consultations are limited to Primary Care services offered at participating Care Center Clinics and MegaClinic branches and are subject to physician availability.
2. The cardholder is entitled to one (1) Annual Physical Examination (APE) during the card's validity period. The APE package includes a Physical Examination, Complete Blood Count (CBC), Urinalysis, Fecalysis, and Chest X-ray only. The Annual Physical Examination benefit is non-transferable, non-convertible to cash, and may not be substituted for other laboratory tests, procedures, or medical services.
3. The cardholder is entitled to a fifteen percent (15%) discount on selected laboratory and diagnostic services offered by the clinic.
4. The cardholder is entitled to a ten percent (10%) discount on eligible medicines purchased at participating VidaCure Pharmacies. This discount cannot be used in conjunction with any other promotion, discount, or special offer.
5. The card may be transferred up to two (2) times within the card's validity period.
- Transfer privileges remain exclusively with the original cardholder. A transferred card may only be returned to the original cardholder and may not be re-transferred, reassigned, or endorsed by the transferee to another immediate family member or any other person.
- Transfers are permitted only to immediate family members, specifically the cardholder's spouse, child, parent (mother or father), or sibling.
- Valid proof of relationship and identification documents must be presented and verified prior to any transfer.
- Upon transfer, the recipient shall be entitled only to the remaining unused benefits associated with the card.
- Benefits already availed by a previous cardholder, including the One-Time Annual Physical Examination (APE), shall be considered consumed and may not be reinstated, repeated, or converted to another benefit.
- The card may not be transferred to individuals outside the immediate family.
6. The CarePlus Flex Card is valid for one (1) year from the date of activation.
7. The card and its benefits are strictly non-transferable and may only be used by the registered cardholder, except as otherwise provided in clause number 5.
8. Valid identification may be required upon registration and every benefit availment.
9. Payments made for the purchase of the Card are final and non-refundable, except where otherwise required by applicable law. The Card and all benefits thereunder are non-convertible to cash and non-exchangeable for other products, services, or membership plans.
10. The clinic reserves the right to refuse, suspend, or terminate card privileges in cases of fraud, misrepresentation, misuse, or violation of these Terms and Conditions.
11. The clinic reserves the right to amend these Terms and Conditions as necessary, subject to applicable laws and regulations.
12. Nature of the Product. The CarePlus Card is a prepaid healthcare membership program that provides access to specified healthcare services and discounts. It is not a health insurance policy, health maintenance organization (HMO) plan, or insurance product and shall not be construed as providing reimbursement, indemnity, or financial coverage for hospitalization or medical expenses beyond the benefits expressly stated herein.
13. Care+Center shall not be liable for the temporary unavailability of services due to force majeure, government regulation, epidemics, internet interruptions, physician unavailability, or circumstances beyond its reasonable control.
14. By enrolling, the Cardholder authorizes Care+Center and its accredited service providers to collect, process, store, and share personal and health information solely for enrollment, verification, benefit administration, and healthcare delivery, in accordance with the Data Privacy Act of 2012.
15. Lost, stolen, or damaged cards may be replaced upon payment of the applicable replacement fee and subject to identity verification.
16. These Terms shall be governed by Philippine law.

How do I activate CarePlus Flex?
After payment confirmation you receive a card number and one-time code by email. Use them to create or link your Care Center patient account. The 1-year validity starts on activation.
